Submit the form with document copies and pay the fee at the office.

Get your seat
Your name goes to the Para Medical Board for registration. We give you the class start date.

- PUC marks card — only if you want lateral entry
- Transfer Certificate from your last school or college
- Study Certificate
- Caste and income certificate — if you are claiming a reservation
- Aadhaar card copy
- 6 passport size photos
